This Change Request (CR) is revising Chapter 1, section 30.2.9 and deleting section 30.2.9.1, of Publication 10004, Claims Processing Manual. In addition, this CR is removing references of the term "purchased diagnostic test" and "purchased test interpretation" and replacing those terms with "antimarkup test" or "diagnostic tests subject to the antimarkup payment limitation."
