This transmittal revises how Medicare systems process revenue code lines submitted with noncovered charges. It also makes miscellaneous enhancements to the Medicare Claims Processing Manual regarding noncovered charges and Medicare systems.
Claims containing an institutional service line submitted with a revenue code that is not valid for Medicare billing should only be returned to the provider if the line is submitted with covered charges or the claim indicates that beneficiary liability may apply.
